How it Works

Here are the steps on how to create a profile, get your alerts, and find a Community Health Center near you using PrevCarePlus.

Add a family member

Click the “+” in the left top corner. You will get this screen:


Received alerts

To see your alerts, tap on the Alerts button in the left bottom corner. You will see this screen:

Click on the High Priority button. You will see a screen similar to this:

When you click on one of the preventive measures, you will see this menu pop up:

If you want to know why you received a particular alert, click on the “Why?” button. You will see a screen like this:

Click on the preventative measure you want to learn more about:


 Take action

To figure out how to take that preventive measure, click on the Take Action button:

“Taken Care Of”

Once you have taken care of your preventative measure (ie…gone to get your blood pressure checked, etc…), click on your alert, then click “Taken Care Of”. This will dismiss the alert from your app until you need it again.


Find Care

In order to find the nearest community health center or doctor’s office that offers that particular preventative care, click on the “Find Care” button from the alerts menu. If your privacy settings/location services for PrevCarePlus are on in your settings menu of your phone, a list of the nearest clinics will pop up.

You can also find other clinics by clicking middle bottom button. You will get choices of how to search for a clinic: “nearest” or by different variables (zip, city, state, name):

Other Features

The “Alert” part of Settings, when clicked on, looks like this:

You can choose how often you would like to receive your alerts, and how you view them: by Alert Priority or by Family Member.
